POST RELEASE CARE

Preventing recidivism

The Post-Release Care Program serves juvenile and adult males ages 15–35 who have recently been released from detention or correctional supervision for substance- and alcohol-related offenses.


It addresses the high risk of relapse and reoffending that occurs immediately after release, when individuals often lack structure, support, and access to stable resources.


The program begins at the point of release and focuses on the first 90 days, providing structured, high-contact support through peer-led recovery, case management, and reintegration planning. Participants receive accountability check-ins and coordinated access to housing, employment, behavioral health, and recovery services.


The goal is to stabilize reentry early, reduce recidivism, and build a sustainable path to independence

THE PROGRAM IMPACT: Participants are held accountable by their peers, which is a  powerful motivator for positive behavior.


KEY BENEFIT: This reduce social isolation and replace “criminogenic” social networks  like having friends who use drugs with a positive “brotherhood” committed to sobriety.

THE PROGRAM IMPACT: Our peer coaches serve as “living proof“ that change is possible, which reduces shame and increases motivation to stay in the program.


KEY BENEFIT: This program emotional support and informational support such as real world strategies for navigating probation/parole that traditional counselors might lack.
